Resource Request & Maintenance Request Forms

The Resource Request and Maintenance Request Forms are available free below to download, modify and print for your Sunday School, Children's Ministry, church administrative office, Human Resources Dept., etc. 24 hours a day, 7 days a week to download at your convenience.

The sample request form for resources is what you normally turn into the church administrative office at least one week before the service or event you need it for. If you need construction paper, pencils, markers, tape, glue or whatever the need is this is the form you would use.

The Maintenance Request form is what you turn into the church administrative office anytime you see the need for maintenance in your office, Sunday School class, etc. This helps the church keep up with what needs to be fixed or repaired and another way to communicate which is extremely important to any church and/or organization.

Communication is an extremely important asset in any  organization including churches, and one of the most efficient ways to communicate within your organization is to use forms, emails, face-to-face communication, meetings, etc. 

The Children's Ministry Resource Request Form for Resources includes:

  • Name and Phone Number of Person Submitting Request
  • Date Submitted
  • Service
  • Class
  • Resources Needed - Example: Need 25 Sheets of Legal Size, Red, Construction Paper or Need Assorted Colors of Pom Pom Balls, Medium Size, About 75
  • Instructions: Please turn in all requests for copies or resources at least one week before the service you need it for. If you need it for a Sunday service, it needs to be turned in no later the Monday before that Sunday service.

The Children's Ministry Maintenance Request Form includes:

  • Name and Phone Number of Person Submitting Request
  • Date Submitted
  • Location - Insert the location of where the maintenance is needed.
  • Type of Maintenance Needed (Explain in Detail) - Insert a detailed summary of exactly what type of maintenance is needed and on what.
  • Choose from these choices: This is Urgent, Needs to be Done in the Next Few Days or Can Be Done Within the Next Few Weeks
