The Request Forms below are a very important part of the
church as a whole. There are 2 forms, 1 is for a simple short prayer or
praise report, and the other one is for requesting a hospital visitation
for someone who desperately needs prayer.
I always state "God is still on the Throne & Prayer Changes Things!"
We've been taught from a child up to pray. This is how we get to know God in such a personal and special way, and at the same time God answers prayers for many individuals in need of prayer. It always gives you a sense of overwhelming joy to help someone who is in need by praying for them and watching God do the work. He hears your prayers so keep up the great work for God by being a Prayer Warrior (very needed in this day and time and is certainly a special ministry). Without prayer warriors in the church I believe there wouldn't be a church.
The Prayer Meeting and Fasting Flyer below is available to customize for your next prayer and fasting church event. Our Pastor called our church on a 21 day fast at the Watch Night New Year service so we decided to put a flyer up on the bulletin and Facebook page to give everyone the opportunity to pray every night and on church days like an hour before church that could. This made for some great prayer meetings!
The Annual Contribution Letter and IRS Contribution Statement Request form below gives your donors the opportunity to request their letter and/or statement and this form includes:
- Church Name and Title of Form
- Statement - The [Insert Church Name] Annual Contribution Letters and IRS/Contribution Statements are distributed upon request. To request your letter or statement, please complete the information below and return this to the Church Administration Office or scan and email it to [Insert Contact Name and Email Address] by [Insert Date and Time].
- Information Needed - Name, Address, Phone and Envelope Number
- Check What You're Requesting - You can check whether you want the Annual Contribution Letter and/or the Annual IRS Contribution Statement.
