Do you ever just wish you could help choose which songs will be sung at church?
This form is great for those members who desperately want to hear their favorite song in church or whatever the event might be but they don't have time to track down the worship leader, etc.
As someone who very much loves to worship through music, having a request form available for my church is very special because I know if I'm feeling a certain song to help someone through a trial they might be going through or maybe I might be going through the trial of my life and just need that certain special song that has been ministering to my situation. If a song is ministering to me and helping me through a trial then I know it's very likely it could very much help the other people going through similar trials.
The chances are if you're not part of the worship team then you have no say in the songs that get sung at every church service or event.
I know a long time ago in the smaller churches they would actually take song requests from their song books. Sometimes they would just shout out the song page number and sometimes they had an elder or deacon go around taking song requests. Picking out a song and having the entire church sing it left you with a very special loving feeling.
The form for Church Song Requests includes:
- Church Name
- Request Form Title
- Statement - "Use this form to request a song for a church service or event."
- Full Name, Email Address, Phone Number to contact you if any questions
- Song Title
- Artist or Band Name
- Link (If you have a website to the song, put it here please.)
- Specific Church Service or Event and Date and Time for Song
- Your Story (Tell us why this song means so much to you.)
- Your Signature
- Request Date
- Statement - "Complete the Song Request form and return it to the Church Administrative Office. If there are any questions regarding this request form please contact: __________________________.
