Field Trip Request and Permission Forms
The free Field Trip Request & Permission Forms for Children's Ministry activities are available to download, customize and print for your church administrative office, Human Resources Department, etc. 24 hours a day, 7 days a week at your convenience.
The Field Trip Permission Request is a Children's Ministry form especially for your Children's Ministry Volunteers and/or Members to fill out prior to the trip. This will allow the church administrative office to plan accordingly (send flyers, bulletins..) after the permission request forms come back signed and ready for the children to leave on their trip.
The Field Trip Sign Up form is a simple form to sign up the
children that is going on your church field trip, and at the same time
getting permission from their parents. This form also allows the people involved in this field trip to have an idea of exactly who is going and who exactly is not.
The Field Trip Permission Slip is a more extensive printable permission slip form that in detail gives the permission from the parent who signs the form to bring the child on the field trip and permission to care for the child in the case of an emergency to be able to obtain emergency medical care if the unexpected event may arise. This permission slip will have to be filled out and signed by each of the children's parent(s) and/or legal guardian.
Field trips are exciting and fun events for the children to enjoy. The church can make this a fun event for the children and at the same time make sure every child is covered by the correct paperwork so no problem might arise that might hinder the church making sure the child is properly taken care of.
When your Children's Ministry at your church begins the process of planning an all day or extended field trip, there are many important details that have to be addressed first such as the availability of the church buses or vans before you make reservations or purchase tickets for your field trip with the children.
